
Family Nurse Practitioner | Addiction Medicine
Heather L. Young, MSN, CRNP, FNP-C, CSAP, serves as a Family Nurse Practitioner at Tranquility Woods, where she provides medical care and support for individuals seeking treatment for substance use disorders and co-occurring mental health conditions.
With more than 20 years of healthcare experience, Heather brings extensive expertise in addiction medicine, primary care, behavioral health, urgent care, and hospital medicine. Her clinical background allows her to address the complex medical needs often associated with addiction while helping clients build a strong foundation for long-term recovery.
At Tranquility Woods, Heather conducts comprehensive medical assessments, manages withdrawal symptoms, develops individualized treatment plans, interprets laboratory and diagnostic results, and provides ongoing medication management throughout the treatment process. She has significant experience working with Medication-Assisted Treatment (MAT), including Methadone and Suboxone, and is skilled in treating the chronic medical conditions that frequently accompany substance use disorders.
Prior to joining Tranquility Woods, Heather spent more than a decade serving in leadership and clinical roles with Grace Medical Center, where she helped integrate primary care services into substance use treatment programs and led initiatives focused on improving patient outcomes, wellness, and care coordination. She also practiced as a Hospitalist Nurse Practitioner and Urgent Care Nurse Practitioner, gaining valuable experience treating both acute and chronic medical conditions across diverse patient populations.
Heather earned her Master of Science in Nursing, Family Nurse Practitioner, from Walden University and holds a Post-Master’s Certificate in Leadership and Management. She is board-certified through the American Association of Nurse Practitioners (AANP) and holds the Certified Supervised Addiction Professional (CSAP) credential.
Known for her compassionate approach and commitment to individualized care, Heather works closely with the clinical and medical teams at Tranquility Woods to ensure each client receives comprehensive, evidence-based treatment that supports lasting recovery and overall wellness.
