Insurance verification

Know what your insurance may cover.

Understand your benefits and possible costs.
What insurance pays depends on your plan, the treatment you need, and your insurance company’s approval. Checking benefits is a useful first step, but it does not guarantee payment.

Not sure what kind of insurance you have?

Tranquility Woods cannot currently accept Medicaid or Medicare. We commonly check preferred provider organization (PPO) and point of service (POS) plans. If you are unsure which type you have, Admissions can help you check your insurance card. Every plan still needs individual verification.

Start your insurance verification.

Use the form to share your insurance details. We can help explain your insurer’s response and answer questions about the benefits check.

Have your card nearby.

You will need the insurance provider name and member or policy number, along with the patient’s name, date of birth and phone number.

After you submit: Please use a phone number where we can reach you. Call Admissions if you’d like a status update or want to ask when to expect a response. Submitting the form does not reserve a place or guarantee coverage.

Insurance plans we can help you check.

We verify benefits for residential, Extended Care, day treatment and outpatient programs. This list is a starting point, not confirmation that your policy covers a particular program. BlueCross BlueShield plans need out-of-network benefits.
